Before filling out a registration form, check you are in our catchment area.
Important Information Before Registering
To receive healthcare from the NHS You must complete a New Patient Registration Form, this can be done online or in person at the practice. So that we can provide you with the best care, we ask for more information than required to register you as a patient. In order to support the timely and accurate processing of your online Patient Registration Form we use third parties to process your data.
Please read our full Patient Privacy Policy document and the details below for more information. To register, click the green ‘Register at Hingham Surgery’ button at the bottom of this page, you will first be asked to check if you live within our catchment area.
Contact Details
Please be advised if you supply us with your mobile number and / or email address, we will take this as consent to be able to contact you for reasons such as:
- To send survey/questionnaires which save directly into your record
- With a link so you can send photos to the clinician of your rash or lump for example
- Ask you to update clinicians on your treatment or wellbeing
- Invite you to health screenings or vaccinations
- Send referrals, letters or summaries
- Contact you if you miss an appointment e.g. at outpatients
- Contact you if you are not able to answer a phone call
- Send you test results or ask you to call to discuss your results
- Send you general public health messages about COVID 19, flu clinics, mental health or wellbeing services
- In relation to research projects, unless you have objected
You can object at any time by getting in touch with us.
If you share a mobile phone with someone, please be mindful that they may see information about your health.
Repeat Medication
For any repeat medication you will need to ring the Surgery 3 working days after registering to book a telephone appointment with a GP to set up your prescriptions. Once set up, your prescriptions can be ordered online via our practice website, by post, or in person. Elderly or housebound patients can also call between 2.00 pm and 4.00 pm (Monday to Friday).

Armed Forces
When filling in this form you will be asked if you served in the Armed Forces. If you did and you provide us with the information asked, we will take this as consent to be able to annotate this on your record.

Birth Certificates
If this registration is for a child, please ensure you provide us with a copy of their full birth certificate.
Patient Participation Group (PPG)
When filling in this form you will be asked if you would like to join our Patient Participation Group. Our Patient Participation Group consists of 8 members, if you answer that you would like to join and all our positions are filled, you may not be contacted.
