Surgery Closure for Staff Training!

As a Practice we need to continuously ensure all of the Practice Team have the necessary skills and knowledge base.

With this in mind, we will be closed on the 1st Wednesday of every month from 1.00 - 2.00 pm.

Should you telephone the surgery during this time, please follow the message instructions.

 

Registering Your New Born Baby

Once your baby is born then it is important that you register the baby as soon as possible in order for their details to be entered on the system before they have their 6-week baby check and are due to start their immunisations at 8 weeks.  In order to register your baby we will need to see their full birth certificate and photographic identity of mother i.e. drivers licence/passport.  Please speak to Reception as baby registration forms differ from adult forms.

Registering a Child

In order to register your child we will need to see their full birth certificate, photographic identity of mother i.e. drivers licence/passport, proof of address i.e. utility bill and, if possible, Red Immunisations Book.  Please speak to Reception as childrens registration forms differ from adult forms.

Please note that in order to register a child with the practice, at least one parent/guardian must also be registered with us.
