Surgery Closure for Staff Training!

As a Practice we need to continuously ensure all of the Practice Team have the necessary skills and knowledge base.

With this in mind, we will be closed on the 1st Wednesday of every month from 1.00 - 2.00 pm.

Should you telephone the surgery during this time, please follow the message instructions.

 

New Patient Registration

We are always pleased to register new patients, but please check to see if you are in the practice's catchment area by clickng the following link:

If you are in our catchment area and wish to complete a registration either call into the practice and see one of the receptionists who will be pleased to provide you with the necessary forms and information or select the 'Registering Online' tab above.

PLEASE NOTE - To register at this practice you are required to present two forms of identification:

  • Proof of identity (photo ID) - i.e. passport, driving licence
  • Proof of address - i.e. utility bill, bank statement

This must be brought to the practice in person along with your completed registration forms.  Your registration with us will not be complete until this documentation has been provided and your details verified.

On completion of the relevant forms and identity checks you will be registered with a named doctor - although you can choose to see any doctor at the practice.  If you would like to know who your registered doctor is, please contact us and we'll be happy to advise you. 

On registering we would like to invite you to make an appointment for a new patient check.

If there is any particular information you would like us to know, including any special requirements, please tell us when you register.
